APPLICATION FOR ADMISSION – Grades PK -12
This document must be completed for all applicants and submitted with a $75.00 non-refundable application fee by January 15 for first consideration. Please make checks payable to Glenelg Country School.

STUDENT QUESTIONNAIRE – Parts One and Two (Grades 4-12), Parts Two and Three (Grades 6-12)
This document must be completed by the student in his or her own handwriting. It should be submitted with the application.

TRANSCRIPT RELEASE FORM – Grades 2-12
This document should be signed by a parent or guardian and given to the registrar (or school secretary) of the student’s current school. The “transcript” should include the most recent report card, the report card from the previous year, and any standardized testing results acquired during the past two years. We ask for the transcripts to be sent to Glenelg Country School between December 1 and February 1. (For late applicants, please submit the most current information).

ENGLISH/LANGUAGE ARTS REFERENCE – Grades 2-12
This document should be given to the student’s current English or Language Arts teacher. Please ask the teacher to complete and mail this reference form to Glenelg Country School after December 1.

MATHEMATICS REFERENCE – Grades 2-12
This document should be given to the student’s current Mathematics teacher. Please ask the teacher to complete and mail this reference form to Glenelg Country School after December 1.

STUDENT RECOMMENDATION FORM – Grades 2-12
This form should be given the student’s principal, guidance counselor, or another teacher who knows the student well enough to provide meaningful information. Please ask the principal, counselor, or teacher to complete and mail this reference form to Glenelg Country School after December 1.

SCHOOL REFERRAL FORM – Pre-K – Grade 1
This document should be given to the child’s current nursery school teacher, daycare provider, or kindergarten teacher (if applicable) to complete and mail to Glenelg Country School after December 1.

PARENT QUESTIONNAIRE – Pre-K and Kindergarten
This document should be completed by a parent or guardian and mailed to Glenelg Country School with the application (or prior to February 1).
